If there is 120 mj18 players how can everyone shotgun start at the same time on one course?

Ratings. The top 72 by rating will be in the D Pool, the lower 48 rated will be in the E Pool.

Then what does it mean when it says mj18 and mj15 shuffle after the 3rd round?

It means after 3 rounds when all the MJ18 and MJ15 players have played identical courses, the pools are reshuffled. So then the 72 best MJ18 scores will be in D Pool with the 48 worst MJ18 scores in E Pool.
